In short: FDA has finalised its Q&A guidance on postapproval manufacturing changes to licensed biosimilar and interchangeable biosimilar products. It finalises the draft of the same title issued on 23 July 2024 and tells applicants what to report, in which category, and what quality data to provide.

What changed

The FDA page lists the guidance as Final (Level 1), dated October 2026, from CDER and CBER, under docket FDA-2024-D-2581. Its Federal Register notice is dated 13 October 2026. The guidance is in two parts: recommendations on reporting categories, and recommendations on product quality data.

Points from the document:

  • Under 21 CFR 601.12, applicants must assess the effect of each change on identity, strength, quality, purity and potency, and report it in the right category: Prior Approval Supplement (PAS) for major changes, CBE-30 or CBE-0 supplement for moderate changes, and annual report for minor changes.
  • For commonly reported changes, it points applicants to existing FDA guidances on specified biological products and to ICH Q12 as a lifecycle framework. It also notes that products licensed under section 351(k) are outside the scope of one of the biologics change guidances, though the principles may still help.
  • For quality data, applicants should follow ICH Q5E on comparability. The extent of data should fit the type of change and its potential effect on quality, safety and efficacy.
  • The finalisation supersedes the recommendation in Q&A I.20 of FDA’s September 2021 guidance “Questions and Answers on Biosimilar Development and the BPCI Act”, which FDA intends to withdraw.

Who it affects

Indian and other manufacturers of biosimilars that hold, or plan to hold, a US licence, and the QA, QC, analytical development, CMC and regulatory teams who plan changes to process, site, scale, equipment or controls.

Key dates

  • 23 July 2024: draft guidance issued (as stated in the FDA page).
  • 13 October 2026: Federal Register notice of the final guidance.
  • Comments on the final guidance can be submitted at any time under docket FDA-2024-D-2581.

What to do now

  • Update your change-control SOP so each proposed change to a biosimilar is mapped to a reporting category and a comparability plan.
  • Check that your comparability protocols cite ICH Q5E and use quality attributes and analytical methods suited to the risk of the change.
  • Replace references to Q&A I.20 of the 2021 guidance in internal documents.
  • This is an example in-house approach, not a regulatory requirement: keep a one-page table of past changes, the category used and the data supplied, to reuse in future submissions.
