FDA Warning Letter: Pfizer Healthcare India (2020)
Contents
In short: FDA’s March 2020 warning letter to Pfizer Healthcare India described concerns with sterility investigations, environmental monitoring, laboratory records and corrective actions. FDA issued a closeout letter in February 2023, saying the firm appeared to have addressed the cited violations while noting that future inspections would assess whether corrections remained effective.
Case timeline
| Date | Event |
|---|---|
| 29 August–6 September 2019 | FDA inspected Pfizer Healthcare India’s drug manufacturing facility in Visakhapatnam, India. |
| 25 March 2020 | FDA issued Warning Letter 320-20-31 (MARCS-CMS 594972). |
| 8 February 2023 | FDA issued a closeout letter after evaluating the firm’s corrective actions. |
Main observations in the 2020 letter
FDA reported that the firm’s investigations and corrective actions did not adequately address several quality-system concerns:
- After a sterility-test failure, the investigation and follow-up did not resolve the suspected equipment and container-integrity issues promptly enough. FDA also questioned the adequacy and timeliness of related corrective and preventive actions.
- The letter described repeated contamination findings in negative control plates and environmental monitoring, along with delays in reading microbiological plates. FDA said results had been invalidated without sufficient scientific justification and that potential product and process impact was not adequately assessed.
- FDA raised concerns about laboratory data accuracy and requested a wider assessment of documentation and data-integrity controls.
The letter cited 21 CFR 211.192 for inadequate investigation of unexplained discrepancies and specification failures. The current text of that section requires a thorough investigation whether or not a batch has been distributed, extension of the review to potentially related batches or products, and a written record of conclusions and follow-up.
Corrective information requested
FDA asked the firm to assess its investigation and CAPA systems, review the extent and potential product impact of data inaccuracies, and develop a management plan for correcting data-integrity controls. It also requested an independent review of laboratory practices, methods, equipment, records, staff capability and workload.
The letter requested follow-up information on the sterility failure, environmental monitoring and delayed plate readings. FDA also asked for records and supporting analysis that could show the scope of affected batches and the effectiveness of corrective actions. The response was requested within 15 working days of receipt of the letter.
Closeout status
In its 8 February 2023 closeout letter, FDA said its evaluation indicated that the firm appeared to have addressed the violations in Warning Letter 320-20-31. The agency said future inspections and regulatory activity would assess the adequacy and sustainability of those corrections. The closeout letter also states that the firm remains responsible for maintaining compliance and that future regulatory action remains possible if new violations are observed.
Practical review point
Example in-house criterion: Before closing a significant investigation, document the affected batches and products considered, the evidence supporting the root cause, the corrective actions taken, and how effectiveness will be checked. For laboratory events, include environmental and control data, reading timelines, raw records and any rationale for treating a result as invalid.
